09V068000 · Structure:Body
Reported: 2009-02-23 (February 23, 2009) · Model years: 2007, 2008, 2009
COLLINS IS RECALLING 559 MY 2007 THROUGH 2009 SCHOOL BUSES BUILT ON FORD E-350 AND E-450 CHASSIS FROM NOVEMBER 3, 2006 TO OCTOBER 1, 2009. THESE BUSES MAY HAVE BEEN PRODUCED WITH THE ABSENCE OF TWO BODY MOUNT BOLTS AND ASSOCIATED HARDWARE IN 2 OF THE BODY/CHASSIS MOUNTING LOCATIONS IMMEDIATELY BEHIND THE REAR AXLE.
Consequence: IF ANY OF THE BODY MOUNT BOLTS ARE MISSING THIS CONDITION WOULD NOT PROVIDE A STABLE CONNECTION OF THE BUS BODY TO THE BUS CHASSIS. IN THE EVENT OF A VEHICLE CRASH, THERE MAY BE AN INCREASED RISK OF PERSONAL INJURY TO THE VEHICLE OCCUPANTS.
Remedy: COLLINS WILL NOTIFY OWNERS AND INSPECT AND REPAIR THE BUSES IF NECESSARY FREE OF CHARGE. THE SAFETY RECALL BEGAN ON FEBRUARY 26, 2009. OWNERS MAY CONTACT COLLINS TOLL-FREE AT 1-800-533-1850.
NHTSA campaign record ↗09V083000 · Structure:Body

COLLINS IS RECALLING 2,741 MY 2008 SCHOOL BUSES MANUFACTURED BETWEEN JANUARY 1, 2008 AND JANUARY 1, 2009. THE BOLTS USED TO MOUNT THE BODY TO THE CHASSIS FRAME RAIL MAY HAVE BEEN IMPROPERLY INSTALLED WHICH COULD LEAD TO FAILURE OF THE BOLTS.
Consequence: FAILURE OF THESE BOLTS WILL RESULT IN INADEQUATE CONNECTION OF THE BUS BODY TO THE BUS CHASSIS.
THE SAFETY RECALL BEGAN ON MARCH 19, 2009. OWNERS MAY CONTACT COLLINS TOLL-FREE AT 1-800-533-1850.
NHTSA campaign record ↗13V469000 · Equipment:Electrical | Electrical System
Reported: 2013-09-25 (September 25, 2013) · Model years: 2009, 2010
Collins Bus Corporation (Collins) is recalling certain model year 2009-2010 Bantam, Super Bantam, Grand Bantam, Mid Bus Guide, Super Guide, and Grand Guide school buses manufactured September 2009 through April 2010, and equipped with A&M Systems-brand electric door actuators. Voltage spikes may damage electrical components and electrical circuits, causing the circuit board to develop high resistance.
Consequence: High resistance may cause the board to overheat, increasing the risk of a fire.
Remedy: Collins will notify owners, and dealers will inspect and, if necessary, replace the circuit board free of charge. The recall began on December 4, 2013. Owners may contact Collins at 1-800-533-1850.

Emerging complaint spikes
Complete months in the latest six months with at least 10 complaints, at least twice the preceding 12-month average, and at least three square-root deviations above that average.
This is a descriptive screening rule, not a statistical defect finding. Publicity and reporting changes can increase submissions.
No complete months meet the spike thresholds for this selection.
Equal-age model-year comparison
Model-wide cumulative reports during the first three or five calendar years, starting with the model year (age 0). Only fully elapsed windows are compared.
2009: 0 complaints · 2010: 0 complaints
| Model year | Through age 0 | Through age 1 | Through age 2 |
|---|---|---|---|
| 2009 | 0 | 0 | 0 |
| 2010 | 0 | 0 | 0 |
Date coverage across each selected cohort
| Year | Incident dates used | Filing date fallbacks | Excluded dates |
|---|---|---|---|
| 2009 | 0 | 0 | 0 |
| 2010 | 0 | 0 | 0 |
Incident date is preferred when valid and no later than filing; otherwise filing date is used. Model year approximates age. Equal observation time does not adjust for sales, distance driven or reporting delays. One complaint linked to multiple model years can appear in each cohort.
