90V158000 · Equipment:Mechanical:Carrier/Rack

Reported: 1990-08-27 (August 27, 1990) · Model years: 1987, 1988, 1989

INCORRECT LUG BOLTS MAY HAVE BEEN USED IN THE MANUFACTURING OF THE SPARE WHEEL CARRIER. THESE LUG BOLTS MAY BREAK ALLOWING THE SPARE WHEEL TO FALL OFF. CONSEQUENCE OF DEFECT: BREAKAGE OF THE LUG BOLTS ALLOWING THE SPARE WHEEL TO FALL TO THE ROAD WAY WILL CREATE A ROAD HAZARD FOR OTHER VEHICLES.

98V167001 · Equipment:Recreational Vehicle/Trailer:Lpg Systems:Furnace

Reported: 1998-07-20 (July 20, 1998) · Model years: 1992, 1993, 1994, 1995, 1996, 1997, 1998

VEHICLE DESCRIPTION: MOTOR HOMES EQUIPPED WITH AQUA-HOT HEATING SYSTEMS, MODEL 431-3 ZONE WITH ENGINE LOOP. FIBERGLASS INSULATING MATERIAL LOCATED UNDER THE BURNER HEAD ON THE FLOOR OF THE HEATER ENCLOSURE, CAN BECOME SOAKED WITH FLUIDS (ETHYLENE GLYCOL AND/OR DIESEL FUEL).

Consequence: IF AN ELECTRICAL SHORT OCCURS, A FIRE COULD RESULT.

Remedy: VEHICLE SYSTEMS INC. WILL CONDUCT THE OWNER NOTIFICATION FOR THIS CAMPAIGN. DEALERS WILL REMOVE THE INSULATION FROM BENEATH THE DIESEL HEATING UNIT AS WELL AS FROM BEHIND THE CONTROL THERMOSTAT AND AIR-CONDITIONING HEATING ELEMENT ACCESS COVERS.

02V025000 · Visibility:Windshield Wiper/Washer

Reported: 2002-01-24 (January 24, 2002) · Model years: 2002

ON CERTAIN MOTOR HOMES, THE WINDSHIELD WIPER CRANK ARM ASSEMBLY BOLT IS TOO SHORT TO COMPLETELY TIGHTEN THE CRANK ARM AROUND THE MOTOR SHAFT. THE ARM COULD LOOSEN FROM THE MOTOR SHAFT, CAUSING THE WIPER NOT TO WORK.

Consequence: FAILURE OF THE WINDSHIELD WIPERS COULD LEAD TO LOSS OF VISIBILITY DURING CRITICAL PERIODS WHEN WIPERS ARE NECESSARY, INCREASING THE RISK OF A CRASH.

Remedy: DEALERS WILL INSPECT THE WINDSHIELD WIPER ASSEMBLY AND CORRECT IF NECESSARY. OWNER NOTIFICATION BEGAN FEBRUARY 6, 2002. OWNERS WHO TAKE THEIR VEHICLES TO AN AUTHORIZED DEALER ON AN AGREED UPON SERVICE DATE AND DO NOT RECEIVE THE FREE REMEDY WITHIN A REASONABLE TIME SHOULD CONTACT FORETRAVEL AT 1-800-955-6226, EXTENSION 254.

03V222000 · Visibility:Windshield Wiper/Washer:Motor

Reported: 2003-06-12 (June 12, 2003) · Model years: 2002, 2003

ON CERTAIN MOTORHOMES EQUIPPED WITH DUAL LINKAGE WINDSHIELD WIPER SYSTEMS. THE WIPER MOTOR SECURING BOLTS MAY NOT HAVE BEEN PROPERLY TORQUED AND SERCURED WITH THREAD LOCKING COMPOUND.

Consequence: THE SECURING BOLTS MAY WORK LOOSE AND ALLOW THE WIPER ARMS TO TRAVEL OUTSIDE THEIR NORMAL PATH DURING OPERATION. THE DRIVER'S VISABILITY MAY BE IMPAIRED, POSSIBLY RESULTING IN A VEHICLE CRASH.

Remedy: CUSTOMERS WILL BE NOTIFIED AND THE EXISTING MOUNTING BOLTS WILL BE REMOVED AND REPLACED WITH NEW BOLTS, LOCK WASHER, AND THREAD LOCKER. OWNER NOTIFICATION BEGAN ON OCTOBER 8, 2003. OWNERS WHO TAKE THEIR VEHICLES TO AN AUTHORIZED DEALER ON AN AGREED UPON SERVICE DATE AND DO NOT RECEIVE THE FREE REMEDY WITHIN A REASONABLE TIME SHOULD CONTACT FORETRAVEL AT 936-564-8367.

Emerging complaint spikes

Complete months in the latest six months with at least 10 complaints, at least twice the preceding 12-month average, and at least three square-root deviations above that average.

This is a descriptive screening rule, not a statistical defect finding. Publicity and reporting changes can increase submissions.

No complete months meet the spike thresholds for this selection.

Equal-age model-year comparison

Model-wide cumulative reports during the first three or five calendar years, starting with the model year (age 0). Only fully elapsed windows are compared.

Incident date is preferred when valid and no later than filing; otherwise filing date is used. Model year approximates age. Equal observation time does not adjust for sales, distance driven or reporting delays. One complaint linked to multiple model years can appear in each cohort.
