Reported: 2000-01-24 (January 24, 2000) · Model years: 1999

VEHICLE DESCRIPTION: HEAVY DUTY TRUCKS. THE ANTILOCK TRACTION RELAY VALVES WERE INCORRECTLY ASSEMBLED BY THE SUPPLIER. AN EXTRA CHECK VALVE WAS INSTALLED WHICH WILL NOT ALLOW THE TRACTION CONTROL SYSTEM TO FUNCTION PROPERLY.

Consequence: THE REAR BRAKES CAN BE APPLIED AUTOMATICALLY DURING A TRACTION EVENT AND NOT RELEASE UNTIL THE AIR SYSTEM PRESSURE IS DEPLETED.

Remedy: DEALERS WILL INSPECT THESE VEHICLES AND ALL ANTILOCK TRACTION RELAY VALVES FOUND DEFECTIVE WILL BE REPLACED.

VEHICLE DESCRIPTION: HEAVY DUTY TRUCKS EQUIPPED WITH STEERING COLUMNS SUPPLIED BY DOUGLAS AUTOTEC. THE STEERING COLUMNS ON THESE VEHICLES MAY HAVE A DEFECTIVE OR DISLOCATED WELD.

Consequence: IF THE STEERING COLUMN BROKE, A COMPLETE LOSS OF STEERING CONTROL COULD OCCUR, INCREASING THE RISK OF A CRASH.

Remedy: DEALERS WILL REPLACE THE LOWER STEERING SHAFT ASSEMBLY.

VEHICLE DESCRIPTION: HEAVY DUTY TRUCKS. THE CHECK VALVES IN THE AIR BRAKE SYSTEM ON THESE VEHICLES DO NOT MEET THE REQUIREMENTS OF FMVSS NO. 121, "AIR BRAKE SYSTEMS."

Consequence: INCREASED BRAKING EFFORT MAY BE REQUIRED.

Remedy: DEALERS WILL INSTALL NEW CHECK VALVES ON THE VEHICLES INVOLVED.

ON SEPTEMBER 11, 2005, THE VICTIM WAS USING THE RENTAL TRUCK TO MOVE ITEMS FROM STORAGE. THE VICTIM DECIDED TO USE THE ATTACHED RAMP (MANUFACTURED BY DEMCO) TO EASE THE LOADING AND UNLOADING OF ITEMS.
