Recall history
5 distinct campaigns affect this selection. Announcement dates below are different from vehicle model years. A campaign may cover multiple years.
05V110000 · Fuel System, Diesel
Reported: 2005-03-24 (March 24, 2005) · Model years: 1998, 1999, 2000, 2001, 2002, 2003
ON TROLLEYS AND TRANSIT COACHES EQUIPPED WITH CUMMINS ISB DIESEL ENGINES, THE FUEL LIFT PUMP COULD FAIL IN A MANNER WHICH RESULTS IN ENGINE FUEL STARVATION, WHICH COULD RESULT IN A STALL CONDITION. POTENTIAL LIFT PUMP FAILURE MODES INCLUDE: BRUSH WEAR OUT, BUSHING FAILURE, ROTOR TAB INSERT FAILURE, OR ANY OTHER FAILURE MODE THAT WOULD CAUSE THE FUEL PUMPING ELEMENT TO STOP SPINNING.
Consequence: IF THE FUEL LIFT PUMP PUMPING ELEMENT STOPS SPINNING AND THE FUEL PUMPING VANES STOP IN A LOCATION WHICH BLOCKS FUEL FLOW THROUGH THE LIFT PUMP, THE ENGINE WILL BE STARVED OF FUEL. FUEL STARVATION COULD CAUSE LOW ENGINE POWER, ENGINE MISFIRE, OR ENGINE STALL. PRIOR TO THE ENGINE STALLING, THE OPERATOR COULD EXPERIENCE LOW ENGINE POWER, ENGINE MISFIRE, AND/OR DASH-MOUNTED FAULT LAMP ILLUMINATION.
Remedy: CUMMINS IS CONDUCTING THE OWNER NOTIFICATION AND REMEDY FOR THIS CAMPAIGN. OWNERS MAY CONTACT CUMMINS AT 1-800-343-7357 OR OPTIMA AT 316-779-7700.
NHTSA campaign record ↗05V167000 · Steering:Hydraulic Power Assist System
Reported: 2005-04-25 (April 25, 2005) · Model years: 2002, 2003, 2004, 2005
ON CERTAIN TRANSIT BUSES WITH CUMMINS ISB02 ENGINES, THE HYDRAULIC LINE COMING OFF OF THE POWER STEERING PUMP IS RUBBING ON TOP OF THE AIR GOVERNOR.
Consequence: THIS RUBBING MAY CAUSE THE HYDRAULIC LINE TO BE RUBBED THROUGH RESULTING IN A LEAK AND A LOSS OF HYDRAULIC FLUID. THIS FLUID COULD IGNITE IF IT COMES IN CONTACT WITH A HOT SOURCE.
Remedy: DEALERS WILL MAIL A MODIFICATION KIT WITH INSTRUCTIONS ON HOW TO INSTALL A BRACKET AND CLAMP THAT WILL LIFT AND SECURE THE LINES AWAY FROM THE AIR GOVERNOR ELIMINATING THE POSSIBILITY OF CHAFFING. THE RECALL BEGAN ON MAY 20, 2005. OWNERS MAY CONTACT OPTIMA AT 1-888-391-1777.
NHTSA campaign record ↗05V358000 · Engine And Engine Cooling:Cooling System:Radiator Assembly
Reported: 2005-08-15 (August 15, 2005) · Model years: 2002, 2003, 2004, 2005
ON CERTAIN TRANSIT BUSES, THERE IS A POSSIBILITY THAT THE HYDRAULIC LINE COMING FROM THE RADIATOR FAN MOTOR TO THE CAC MOTOR IS RUBBING ON THE CHASSIS.
Consequence: THIS RUBBING MAY CAUSE THE HYDRAULIC LINE TO BE RUBBED THROUGH RESULTING IN A LEAK AND A LOSS OF HYDRAULIC FLUID. THIS FLUID COULD IGNITE IF IT COMES IN CONTACT WITH A HOT SOURCE.
Remedy: DEALERS WILL INSTALL A P-CLAMP ON THE RADIATOR MOTOR SUPPORT BRACKET THAT HOLDS AND SECURES THE HOSE AWAY FROM THE CHASSIS ELIMINATING THE POSSIBILITY OF CHAFFING. THE RECALL BEGAN ON AUGUST 24, 2005. OWNERS MAY CONTACT OPTIMA AT 1-888-391-1777.
NHTSA campaign record ↗05V029000 · Steering:Linkages:Tie Rod Assembly
Reported: 2005-02-04 (February 4, 2005) · Model years: 2004
ON CERTAIN TRANSIT BUSES EQUIPPED WITH ARVINMERITOR URRESKO TIE ROD CLAMPS, HEAT TREATMENT OF CLAMPS CREATED A SURFACE HARDNESS THAT COULD LEAD TO A FRACTURE OF THE CLAMPS.
Consequence: IN THE EVENT OF A CRACK THE CLAMPS COULD BREAK OFF AFFECTING TOE-IN AND ALIGNMENT. THE TIE ROD CAN COME LOOSE RESULTING IN STEERING LOSS AND A CRASH COULD OCCUR WITHOUT PRIOR WARNING.
Remedy: ARVINMERITOR IS CONDUCTING THE OWNER NOTIFICATION AND REMEDY FOR THIS CAMPAIGN. OWNERS MAY CONTACT ARVINMERITOR AT 1-866-668-7221 OR OPTIMA AT 316-779-7700.
NHTSA campaign record ↗05V405000 · Fuel System, Other:Delivery:Fuel Pump
Reported: 2005-09-13 (September 13, 2005) · Model years: 2004, 2005
ON CERTAIN TRANSIT VEHICLES AND TROLLEYS EQUIPPED WITH CUMMINS ISB-02 DIESEL ENGINES BUILT WITH EMC CONTROLLED FUEL LIFT PUMPS. ERRATIC VOLTAGE SUPPLY TO THE FUEL LIFT PUMP CAUSES PREMATURE ELECTRIC BRUSH WEAR INTERNAL TO THE FUEL LIFT PUMP. THIS PREMATURE WEAR COULD RESULT IN FUEL LIFT PUMP FAILURE AND IN SOME CASES MAY CAUSE AN ENGINE STALL CONDITION.
Consequence: SHOULD THE ENGINE STALL, A VEHICLE CRASH COULD OCCUR.
Remedy: CUMMINS, IN CONJUNCTION WITH OPTIMA BUS, WILL NOTIFY OWNERS AND REPAIR THE ENGINES BY EMC RECALIBRATION AND REPLACEMENT OF THE FUEL LIFT PUMP AT NO CHARGE TO THE CUSTOMER. OWNERS MAY CONTACT CUMMINS ASSISTANCE CENTER AT 1-800-DIESELS OR OPTIMA BUS AT 1-316-779-7700.

Emerging complaint spikes
Complete months in the latest six months with at least 10 complaints, at least twice the preceding 12-month average, and at least three square-root deviations above that average.
This is a descriptive screening rule, not a statistical defect finding. Publicity and reporting changes can increase submissions.
No complete months meet the spike thresholds for this selection.
Equal-age model-year comparison
Model-wide cumulative reports during the first three or five calendar years, starting with the model year (age 0). Only fully elapsed windows are compared.
2004: 0 complaints · 2005: 0 complaints
| Model year | Through age 0 | Through age 1 | Through age 2 |

| 2004 | 0 | 0 | 0 |
| 2005 | 0 | 0 | 0 |
Date coverage across each selected cohort
| Year | Incident dates used | Filing date fallbacks | Excluded dates |
|---|---|---|---|
| 2004 | 0 | 0 | 0 |
| 2005 | 0 | 0 | 0 |
Incident date is preferred when valid and no later than filing; otherwise filing date is used. Model year approximates age. Equal observation time does not adjust for sales, distance driven or reporting delays. One complaint linked to multiple model years can appear in each cohort.
