The 2021 RAV4 Prime has a known defect with the heat pump causing an alarming sound to be heard within about a 1 minute of startup. The grinding noise causes distraction as it is extremely loud and occurs generally while backing out/maneuvering in tight areas. Toyota released a Service Bulletin in their European Market in October (numbered BE-00612T-TME - "Train Horn noise" from A/C compressor when the vehicle is started), but has not made available the same fixes in the US even months later.

The primary instrument display started to go out when it was cold, below 50'F. This could be mitigated by running the engine or running the defrost for a period of time to warm up the area around the display. When it was too cold this winter, that was no longer effective and the display would not come on at all. The car would drive and all of the instruments displayed on the screen would work (cruise control, etc.), but there was no way to know how fast I was going and if there were any warning lights illuminated. A few months ago I experienced multiple instances of my car self-starting while parked in my garage. I contacted the dealer and then Toyota. In April I received an email/letter stating that the problem had been corrected. However, just this week the problem returned, starting twice two days ago. The Toyota pre-collision braking system is too slow to release control of the throttle back to the driver. Multiple times it has made the car behave in an unexpected fashion in traffic. The most common scenario is when the driver is stopped and turning left onto a busy street. When a car passes in front of you and you try to accelerate after it passes, the pre-collision braking still thinks there's a car in front of you. Instead of acceleration your car just gently rolls into the street. The driver can't do anything in the moment to override…

This sudden explosive noise put the driver, and all other vehicles on the road, in danger due to the panicked reflex of the driver. Upon inspection, it's evident that the glass shattered along all 4 edges rather than from a single point of impact. This would indicate an issue with the glass, the installation, or both. Vehicle was taken to dealer and customer was told it would not be covered under warranty.

The primary instrument display started to go out when it was cold, below 50'F. This could be mitigated by running the engine or running the defrost for a period of time to warm up the area around the display. The car would drive and all of the instruments displayed on the screen would work (cruise control, etc.), but there was no way to know how fast I was going and if there were any warning lights illuminated. My girlfriend's father was driving at normal driving speed on the highway when the tire blew out for no reason. There are multiple blown out sections of the tire on both the internal and external sides of the tire that would appear to be a structural failure. This was definitely not a puncture due to the failures completely around the entire circumference on both sides of the tire. The tire was inflated as per spec. This caused vehicle instability and was a dangerous situation so he had to quickly pull to the side of the highway. The tire …

The dashboard screen (which shows all data, including speed, gas, charge, etc) has been completely blank/black regularly and will stay that way for days. There is no way to know how fast we are going, how much charge the car has, if there are engine issues, etc. This has been happening for a few months now. The car has not yet been to the dealer to be seen but will be soon. I found many reports of this same thing in the same models and years on reddit and other places on the internet.

The RAV4 Prime's fuel system holds 14.5 gallons, however only between 10 - 11 gallons of that is usable per the digital "miles to zero" gauge on the instrument panel. When the gauge reaches "zero" and notifies the driver to fill up, the tank only fills 10 -11 gallons and keeps 3 - 4.5 gallons in reserve. This is a big issue because the "miles to zero" gauge reads the full tank when you fill it up (approximately 550 miles), but begins to swing drastically on miles left as you drive the car to accommodate for the 10 - 11 gallons that it actuall…

The instrument cluster (combination meter) failed completely and without warning, going entirely blank. This eliminated all speedometer, warning light, and indicator functionality. The failure began occurring intermittently before becoming persistent. No warning lamps or messages preceded the failure , the cluster simply went dark. The loss of speedometer functionality made the vehicle unsafe to operate, as the driver had no means of monitoring vehicle speed. The loss of all warning indicators eliminated visibility into any safety-critical vehi…

We have COMPLETELY lost brakes on MULTIPLE occasions for a few seconds per occasion. Brake loss has occurred upon slow, even braking and on flat, dry, even pavement with no pot poles or loose gravel around. When this happens, an orange light appears on the dash, we lose brakes for a few seconds, the car regains brakes, and jolts forward with no change in foot position on the brake pedal. We have taken the car to the dealer multiple times and every single time this incident has occurred the rear right wheel speed shows about a 10% variation in s…

I was pulling into a parking space at a slow speed and breaking when my car accelerated violently and caused me to go across a median and strike another parked vehicle. Police came and did a report. I took my car right away to dealer and they had me take it to their collision shop to be repaired. They said they would try to find the unintended acceleration problem after it was repaired. No warning lights or airbags.

Driver was parking the car in a parking lot at a very slow speed. The car suddenly shot forward at a very high rate of speed, requiring evasive steering from trees and a fence, until the car crashed head-on into another vehicle. The driver is very certain the brakes only were applied during the entire time the car shot forward. Airbags were not deployed. Bystanders were within six feet of the car at one point during the acceleration, prior to the crash. There were no injuries. Earlier, on the day of the crash, the car had a sudden, stro…

The driver was pulling into a friend's carport. After crossing a small curb her foot was taken off the gas and was moving to the brake. The car was going slightly uphill as slow as a car can go with no gas pedal depressed. The car then accelerated as if someone pressed the gas to the floor. In a matter of 8 feet it gained enough speed and momentum that when it hit our friend's parked car the 4 door SUV was pushed forward 4 feet. The gas pedal was never pushed by the driver. No failed component was ever identified by multiple inspections by the …

The digital instrument cluster display failed completely while the vehicle was in operation. The speedometer, warning indicators, and safety alerts were not visible, making it unsafe to drive due to inability to monitor speed or receive critical warnings. There were no prior warning messages before the failure occurred. The vehicle was inspected by an authorized Toyota dealer, Toyota of Temecula, who confirmed instrument cluster failure. The vehicle has been out of service since November 25, 2025. The dealership has ordered a replacement instr…

While exiting an interstate and applying the brakes, the vehicle struck a small bump (pot-hole) in the roadway with the front passenger wheel. The car lurched forward and stopped slowing down. The brakes had disengaged on all wheels. After approximately 2 seconds, the car started braking again and I was able to regain control and stop. This momentary loss of braking ability could have caused me to rear end the vehicle in front had I not left excess room. The loss of brakes increased the braking distance required and could lead to the loss …

All four brake rotors are defective, the metal is pitted scored and not smooth, as it should be. This increases the braking distance. It is the HV floor wire connector, when the corrosion is beyond repair, the parts have been back ordered for months. Toyota and dealerships are not approving loaner vehicles.
