My vehicle was slowing down in the carpool lane and I had to carefully find my way to the shoulder to be able to turn the vehicle EV motor back on. There are no warning signs. It is very sudden. I was stopped at a traffic light for several minutes. I started to feel like we were moving and checked the brakes to make sure they were still fully engaged. When I realized that they were and that I was still rolling, I put the car into park. I did bump the semi in front of me. I’m not sure if the impact or putting the car in park that stopped me. 7 distinct campaigns affect this selection. Announcement dates below are different from vehicle model years. A campaign may cover multiple years.

22V181000 · Seats:Mid/Rear Assembly

Reported: 2022-03-24 (March 24, 2022)

Chrysler (FCA US, LLC) is recalling certain 2021-2022 Pacifica and 2021 Voyager vehicles. The second-row seat-to-floor mounting latch may bind and stick open, preventing the seat from properly securing to the floor. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 225, "Child Restraint Anchorage Systems."

Consequence: An unsecured seat increases the risk of injury in a crash.

Remedy: Dealers will inspect and, if necessary, replace the second-row seat cushion frame, free of charge. Owner notification letters were mailed April 7, 2022. Owners may contact FCA US, LLC customer service at 1-800-853-1403. FCA US LLC's number for this recall is Z22.

CAR SHUTS OFF WHILE DRIVING. WARNINGS TO PUT THE CAR IN PARK AND ELECTRONIC STABILITY ISSUES MAY POP UP. THE ENGINE MAKES A WEIRD NOISE AND THE CAR WILL OFTEN NOT SHIFT TO PARK TO RESTART. THIS HAS OCCURRED THREE SEPARATE OCCASIONS FOR ME. EACH TIME I WAS LUCKILY DRIVING SLOWLY PRIOR TO A TURN. I HAVE BROUGHT THE CAR TO THE DEALER EACH TIME. THEY HAVE TRIED REPLACING THE SHIFTER, UPDATED THE COMPUTER SYSTEM AND CLEARED ERROR CODES. NOTHING WORKS. THIS IS A HUGE SAFETY ISSUE FOR OUR FAMILY AND THEY JUST KEEP RETURNING THE CAR TO ME.

I was driving uphill at slow speed; then a garbage truck in front of me needed to do a 3 point turn so I switched to reverse to back up. At that point the vehicle started flashing an alarm, I think said "service hybrid system" or something like that and instantly began rolling down the hill for about a block, while I was completely unable to engage the break or gas, to switch to drive, to control the wheel, even the emergency brake failed. I was rolling backwards for about a block completely unable to control any component of the car when th…

My own words describing the incident: We have a new 2021 PHEV Limited (purchased May 3, 2021), which has with 977 miles (554.5 electric, 423.1 Hybrid). On May 24, 2021 my wife had driven about 2 miles to a store parking lot, turned off the car and parked, then came back home, all on electric, all working fine. I was driving in stop and go traffic about 4 hours into a road trip on I95. At around 60mph, an error popped up on the dash saying “Service Charging System” then it quickly went away and said “Shift to P to Start” at which point I lost all propulsion. I was in the left lane and had to make my way to the side of the road with my family of 6 in the car. This was extremely stressful and dangerous as I had no way to accelerate and was stuck to coast. I flipped on my hazard lights and was able to navigate to a safe place under an overpass. At this p…

The engine stopped abruptly on an interstate. We were required to slow from 75 mph and pull off on the side of the road immediately. The transmission changed to what felt like first gear before we were adequately slowed down, grinding and making a lot of noise. This happened on 3 separate occasions before the dealership could figure out the problem. If we had been in a situation where we couldn’t have pulled off the road immediately, it would have been much more dangerous. The engine stops and will not let you accelerate. The dealership is now…

Vehicle has completely lost power multiple times while driving on the freeway at 65 approximately MPH and displays "Service Charging System" message on dash. Accelerator pedal was completely non-responsive. Have had to quickly try to get to side of freeway and stop. Each time the van had remaining fuel. Each time we have been able to get the van started again by putting the vehicle in Park and pressing the start button 3 times in a row without pressing the brake pedal. It is very concerning and has happened to us several times this month with…

My PHEV Pacifica Hybrid experiences intermittent dangerous lurching during braking. Symptoms: - During modest braking where regenerative braking system is doing most of the braking, the regenerative braking system flutters on an off a couple seconds at a time - The Power gauge on the cluster (Far Left) Bounces wildly with each flutter - Sudden loss of braking causes lurching of vehicle - Problem is periodic. Will not have the problem for weeks and then suddenly have the issue constantly for several days I have experienced this at highways spe…

On 12/20/2024 my wife was driving about 20mph on a road and with no warning lights or messages the vehicle completely shut down. Nothing would work on the vehicle. She could not even put my hazard lights on or put the vehicle in neutral to move out of the roadway. The vehicle blocked traffic for an hour. It was fortunate the car behind did not hit our vehicle and also that my children were not in the car with her. The police assisted in redirecting traffic around the vehicle but they also were unable to get anything working. A tow truck towed t…

Van has 70K miles. After a long road trip. last stop noticed a gas smell from the front of van. When opening the hood noticed gas had been leaking over top of the motor by spraying fuel. Moving the fuel line running over the top of the motor, showed it was rubbing against top of the coil pack on engine. The rubbing over time caused a small cut in the fuel line allowing for fuel leak. The fuel line seam to be malformed or is unsported causing the rubbing.

Van had a smell of gasoline inside the cabin and out. Upon inspection, the fuel line going from the hard line to the nylon line in the engine bay was leaking. The nylon line between the fuel pressure sensor and the hard line had rubbed against a coil pack and was leaking fuel. Any fuel leak, especially in the engine bay, poses a safety risk as it can combust and cause a fire inside the engine bay. Other owners have posted about this exact problem on forums, hence why I knew where to look for the issue. I have ordered the new fuel line thro…

While driving my children home from an evening activity, there was a strong odor of gas inside the cabin. I had the vehicle towed to the dealership the next morning for diagnostics and repairs. The dealership 's technician was able to reproduce the issue and upon further inspection discovered a small leak in the fuel line. It was determined that the fuel line had been rubbing against the coil pack, which created a small hole. The fuel was leaking onto the coil pack. This puts both the driver and the passengers at risk because the coil pack is i…
