Recall history
6 distinct campaigns affect this selection. Announcement dates below are different from vehicle model years. A campaign may cover multiple years.
20V546000 · Power Train:Axle Assembly | Suspension:Rear:Springs:Leaf Spring Assembly:U-Bolt, Leaf Spring To Axle
Reported: 2020-09-09 (September 9, 2020)
E-One Incorporated (E-One) is recalling certain 2018-2020 Cyclone 2 and Typhoon ambulances. The lower shock mount bracket may cause the U-bolts to bind and break, possibly preventing the rear axle from remaining secured.
Consequence: The unsecured rear axle may shift sideways, resulting in loss of control of the vehicle, thereby increasing the risk of crash.
Remedy: E-One has notified owners, and dealers replace any of the affected lower shock mount brackets, free of charge. The recall began September 21, 2020. Owners may contact E-One customer service at 1-888-237-1751.
NHTSA campaign record ↗20V752000 · Electrical System: Instrument Cluster/Panel
Reported: 2020-12-04 (December 4, 2020)
E-One Incorporated (E-One) is recalling certain 2020-2021 Commercial, 2019-2020 Cyclone 2 and 2020 Cyclone N, Quest 2, Typhoon and Typhoon N vehicles, equipped with Weldon V-Mux Vista IV Standard LCD displays used for controlling emergency vehicle lighting and HVAC functions. These displays may experience low voltage spikes, causing the LCD display to lock up or become nonfunctional.
Consequence: A locked up or non-functional LCD display can delay emergency efforts, increasing the risk of injury.
Remedy: E-One will notify owners, and dealers will replace the display or apply a retrofit remedy, free of charge. E-One issued owners an interim notification on December 15, 2020. The recall began January 26, 2021. Owners may contact E-One customer service at 1-352-237-1122.
NHTSA campaign record ↗21V099000 · Steering:Critical Fasteners | Suspension:Front
Reported: 2021-02-19 (February 19, 2021)
E-One Incorporated (E-One) is recalling certain 2020 Cyclone 2, Typhoon, and Typhoon N vehicles. The steering gear mounting bolts may become loose and back out, resulting in difficulty steering.
Consequence: Difficulty steering, or the inability to steer in both directions, can increase the risk of a crash.
Remedy: E-One will notify owners, and dealers will replace the steering gear mounting bolts and washers, free of charge. The recall is expected to begin March 2021. Owners may contact E-One customer service at 1-352-237-1122.
NHTSA campaign record ↗21V332000 · Fuel System, Diesel:Fuel Injection System:Fuel Rail
Reported: 2021-05-11 (May 11, 2021)
E-One Incorporated (E-One) is recalling certain 2017-2021 Cyclone 2, Cyclone N, Quest 2, Typhoon, and Typhoon N vehicles. The sealing washer may not seat correctly in the pilot bore holes, allowing the high pressure fuel rail assembly to leak.
Consequence: A fuel leak in the presence of an ignition source increases the risk of a fire.
Remedy: Dealers will inspect and replace the rail as necessary, free of charge. Owner notification letters were mailed out on May 28, 2021. Owners may contact E-One customer service at 1-352-237-1122.
NHTSA campaign record ↗21V952000 · Fuel System, Diesel:Fuel Injection System:Fuel Rail
Reported: 2021-12-09 (December 9, 2021)
E-One Incorporated (E-One) is recalling certain 2019-2021 Cyclone 2, Quest 2, 2020-2021 Typhoon N, 2020 Typhoon, and 2021 Cyclone N vehicles. The fuel tubes between the fuel rail and the injectors for cylinders four, five, and six may fatigue and crack, which can result in a high pressure fuel leak.
Consequence: A high pressure fuel leak in the presence of an ignition source can increase the risk of a fire.
Remedy: Dealers will replace the fuel tubes and install vibration isolators, free of charge. Owner notification letters were mailed February 14, 2022. Owners may contact E-One customer service at 1-352-237-1122.

The neutral screw connection within the QO Plug-On Neutral Load Center may be loose. If that connection is loose, the affected load center can overheat.
Consequence: An overheated load center can increase the risk of a fire.
Remedy: Dealers will inspect and repair, or replace the load center, free of charge. Owner notification letters were mailed October 24, 2022. Owners may contact E-One's customer service at 1-352-237-1122.

Emerging complaint spikes
Complete months in the latest six months with at least 10 complaints, at least twice the preceding 12-month average, and at least three square-root deviations above that average.
This is a descriptive screening rule, not a statistical defect finding. Publicity and reporting changes can increase submissions.
No complete months meet the spike thresholds for this selection.
Equal-age model-year comparison
Model-wide cumulative reports during the first three or five calendar years, starting with the model year (age 0). Only fully elapsed windows are compared.
2021: 0 complaints · 2022: 0 complaints
| Model year | Through age 0 | Through age 1 | Through age 2 |
|---|---|---|---|
| 2021 | 0 | 0 | 0 |
| 2022 | 0 | 0 | 0 |
Date coverage across each selected cohort
| Year | Incident dates used | Filing date fallbacks | Excluded dates |
|---|---|---|---|
| 2021 | 0 | 0 | 0 |
| 2022 | 0 | 0 | 0 |
Incident date is preferred when valid and no later than filing; otherwise filing date is used. Model year approximates age. Equal observation time does not adjust for sales, distance driven or reporting delays. One complaint linked to multiple model years can appear in each cohort.
