05V090000 · Wheels
Reported: 2005-03-14 (March 14, 2005)
ON CERTAIN CLASS B AND CLASS C MOTOR HOMES, THE SPARE WHEELS CONTAIN AN INCORRECT OFFSET. THE CENTER OF THE WHEEL WAS WELDED IN THE WRONG POSITION.
Consequence: THE WHEEL MAY RUB AGAINST THE BRAKE CALIPER WHICH COULD RESULT IN A BRAKE FAILURE INCREASING THE RISK OF A CRASH.
Remedy: THE SPARE WHEELS WILL BE REPLACED FREE OF CHARGE. THE RECALL BEAGN ON MAY 13, 2005. OWNERS MAY CONTACT FOUR WINDS AT 1-574-266-1111.
NHTSA campaign record ↗05V272000 · Equipment:Other:Labels
Reported: 2005-06-10 (June 10, 2005)
CERTAIN MOTOR HOMES BUILT ON FORD E-450 CHASSIS FAIL TO COMPLY WITH THE REQUIREMENTS OF FEDERAL MOTOR VEHICLE SAFETY STANDARD NO. 120, 'TIRE SELECTION AND RIMS FOR MOTOR VEHICLES OTHER THAN PASSENGER CARS.' THE TIRE PRESSURE IS INCORRECTLY LISTED ON THE LABEL FOR THE REAR TIRES.
Consequence: AT THE INCORRECTLY STATED PRESSURE, THE REAR TIRES ARE UNABLE TO SUPPORT THE GROSS AXLE WEIGHT RATING (GAWR) FOR THE REAR AXLE.
Remedy: OWNERS AND DEALERS WILL BE MAILED SUPPLEMENTARY TIRE LABELS CONTAINING THE CORRECT REAR TIRE INFLATION INFORMATION. OWNERS MAY CONTACT FORD AT 1-800-392-3673 OR FOUR WINDS AT 1-574-266-1111.
NHTSA campaign record ↗06V351000 · Engine And Engine Cooling
Reported: 2006-09-13 (September 13, 2006)
ON CERTAIN MOTOR HOMES BUILT ON WORKHORSE CHASSIS AND EQUIPPED WITH GM 8.1L V8 ENGINES, IMPROPER HARDENING DURING THE HEAT TREATMENT PROCESS CAN CAUSE THE FUEL RAIL PULSE DAMPER RETAINER CLIP TO FRACTURE, RESULTING IN INADEQUATE RETENTION OF THE DAMPER. IF THE DAMPER BECOMES LOOSE, FUEL COULD BE PUMPED BY THE FUEL PUMP INTO THE UNDER HOOD AREA.
Consequence: IN THE PRESENCE OF AN IGNITION SOURCE, A FIRE COULD RESULT.
Remedy: CHEVROLET AND WORKHORSE ARE HANDLING THE OWNER NOTIFICATION AND REMEDY FOR THIS CAMPAIGN (PLEASE SEE 06V225 AND 06V289). DEALERS WILL REPLACE THE FUEL RAIL PULSE DAMPER RETAINER CLIPS ON THESE ENGINES FREE OF CHARGE. OWNERS MAY CONTACT CHEVROLET AT 1-800-630-2438, WORKHORSE AT 877-294-6773 OR THOR INDUSTRIES AT 937-596-6849.

Emerging complaint spikes
Complete months in the latest six months with at least 10 complaints, at least twice the preceding 12-month average, and at least three square-root deviations above that average.
This is a descriptive screening rule, not a statistical defect finding. Publicity and reporting changes can increase submissions.
No complete months meet the spike thresholds for this selection.
Equal-age model-year comparison
Model-wide cumulative reports during the first three or five calendar years, starting with the model year (age 0). Only fully elapsed windows are compared.
2010: 0 complaints · 2011: 0 complaints
| Model year | Through age 0 | Through age 1 | Through age 2 |

Date coverage across each selected cohort
| Year | Incident dates used | Filing date fallbacks | Excluded dates |
|---|---|---|---|
| 2010 | 0 | 0 | 0 |
| 2011 | 0 | 0 | 0 |
Incident date is preferred when valid and no later than filing; otherwise filing date is used. Model year approximates age. Equal observation time does not adjust for sales, distance driven or reporting delays. One complaint linked to multiple model years can appear in each cohort.
