10V354000 · Equipment Adaptive/Mobility
Reported: 2010-07-30 (July 30, 2010)
TURTLE TOP IS RECALLING CERTAIN MODEL YEAR 2009 TRANSIT AND SHUTTLE BUSES EQUIPPED WITH CERTAIN RICON S SERIES AND K SERIES L-MODEL WHEELCHAIR LIFTS. THE WHEELCHAIR LIFTS CONTAIN DEFECTIVE SOFTWARE THAT CONTROLS THE LIFT UP FUNCTION. IF THE LIFT SWITCH IS RELEASED BEFORE THE FLOOR LEVEL LIMIT SWITCH IS ACTIVATED, THE LIFT MAY CONTINUE ITS UPWARD TRAVEL FOR APPROXIMATELY 1 SECOND BEFORE STOPPING, AND MAY TRAVEL PAST THE FLOOR LEVEL.
Consequence: WHEN THE PLATFORM OVERRUNS THE VEHICLE FLOOR LEVEL AND STOPS AT AN ANGLE FROM 15-20 DEGREES, IT IS POSSIBLE FOR A PERSON IN A WHEELCHAIR TO TIP OVER AND FOR SOMEONE STANDING ON THE PLATFORM TO FALL, POSSIBLY RESULTING IN INJURIES.
Remedy: TURTLE TOP IS WORKING WITH RICON AND THE REPAIRS WILL BE PERFORMED BY AUTHORIZED RICON DEALERS. PLEASE SEE RICON'S DEFECT REPORT 09E-061. RICON WILL PROVIDE OWNERS WITH A BULLETIN OUTLINING PROPER WHEELCHAIR LIFT OPERATION AND WILL SHIP OWNERS A DVD-BASED TRAINING AID TO PROMOTE PROPER LIFT OPERATION. THE SAFETY RECALL BEGAN ON OCTOBER 8, 2010. OWNERS MAY CONTACT RICON CUSTOMER SERVICE AT 1-800-322-2884, OR EMAILING AT [email protected].

THE FIRST AID KITS COULD POSSIBLY CONTAIN CONTAMINATED POVIDINE IODINE PREP PADS.
Consequence: CONTAMINATED PADS COULD LEAD TO LIFE THREATENING INFECTIONS IN AT-RISK INDIVIDUALS.
Remedy: TURTLE TOP WILL NOTIFY OWNERS AND THE AFFECTED PREP PADS SHOULD BE DISCARDED. THE IODINE PREP PADS WILL BE REPLACED FREE OF CHARGE BY CERTIFIED SAFETY MANUFACTURING, INC. OWNERS SHOULD CONTACT CERTIFIED SAFETY AT 1-800-854-7474 TO OBTAIN THE FREE REPLACEMENT PADS. THE SAFETY RECALL BEGAN ON APRIL 15, 2011.

Emerging complaint spikes
Complete months in the latest six months with at least 10 complaints, at least twice the preceding 12-month average, and at least three square-root deviations above that average.
This is a descriptive screening rule, not a statistical defect finding. Publicity and reporting changes can increase submissions.
No complete months meet the spike thresholds for this selection.
Equal-age model-year comparison
Model-wide cumulative reports during the first three or five calendar years, starting with the model year (age 0). Only fully elapsed windows are compared.
2010: 0 complaints · 2011: 0 complaints
| Model year | Through age 0 | Through age 1 | Through age 2 |
|---|---|---|---|
| 2010 | 0 | 0 | 0 |
| 2011 | 0 | 0 | 0 |
Date coverage across each selected cohort
| Year | Incident dates used | Filing date fallbacks | Excluded dates |
|---|---|---|---|
| 2010 | 0 | 0 | 0 |
| 2011 | 0 | 0 | 0 |
Incident date is preferred when valid and no later than filing; otherwise filing date is used. Model year approximates age. Equal observation time does not adjust for sales, distance driven or reporting delays. One complaint linked to multiple model years can appear in each cohort.
