Reported: 2001-02-05 (February 5, 2001)

VEHICLE DESCRIPTION: HEAVY DUTY TRUCKS EQUIPPED WITH AN OPTIONAL FIREWALL-MOUNTED CIRCUIT BREAKER. THE MAIN CAB POWER AND BATTERY POWER LEADS WERE ATTACHED INCORRECTLY. THIS CONDITION CAN RESULT IN THE MAIN CAB POWER BEING UNPROTECTED.

Consequence: IF A WIRING SHORT OCCURRED ANYWHERE ALONG THE CAB POWER CIRCUIT, THE AVAILABLE BATTERY POWER COULD ARC AND CAUSE SUBSEQUENT DAMAGE TO THE VEHICLE OR A FIRE.

Remedy: DEALERS WILL INSPECT AND CORRECT THE PROBLEM, AS NEEDED. OWNER NOTIFICATION BEGAN FEBRUARY 26, 2001. OWNERS WHO TAKE THEIR VEHICLES TO AN AUTHORIZED DEALER ON AN AGREED UPON SERVICE DATE AND DO NOT RECEIVE THE FREE REMEDY WITHIN A REASONABLE TIME SHOULD CONTACT KENWORTH AT 1-425-828-5000.

01V051000 · Suspension:Front:Control Arm:Lower Arm

Reported: 2001-02-20 (February 20, 2001)

VEHICLE DESCRIPTION: VEHICLES EQUIPPED WITH 54" REAR AXLE SPACING CONFIGURATION OF THE AIRGLIDE 200 SUSPENSION WITH THE SERVICE APPLICATION IDENTIFIED AS LOGGER, DUMP TRUCK, OR TANKER. THE REAR SUSPENSION IS REGULARLY SUBJECTED TO EXTREME ARTICULATION, RESULTING IN FLEXING OF THE JOINT.

Consequence: FAILURE OF THE LOWER SWAY ARM ATTACHING BOLTS COULD RESULT IN LOSS OF VEHICLE CONTROL.

Remedy: DEALERS WILL REPLACE THE LOWER SWAY ARM BRACKETS. OWNER NOTIFICATION BEGAN MARCH 27, 2001. OWNERS WHO TAKE THEIR VEHICLES TO AN AUTHORIZED DEALER ON AN AGREED UPON SERVICE DATE AND DO NOT RECEIVE THE FREE REMEDY WITHIN A REASONABLE TIME SHOULD CONTACT KENWORTH AT 1-425-828-5000.

01V305000 · Visibility:Defroster/Defogger/Hvac System

Reported: 2001-09-26 (September 26, 2001)

VEHICLE DESCRIPTION: CERTAIN HEAVY DUTY TRUCKS FAIL TO COMPLY WITH THE REQUIREMENTS OF FMVSS NO. 103, "WINDSHIELD DEFROSTING AND DEFOGGING SYSTEMS." THE MODE DOOR IN AN HVAC SYSTEM CAN BECOME INOPERABLE. THE MODE DOOR CAM AND ACTUATOR COULD BIND, CAUSING THE CAM RETAINING TABS TO SHEAR OFF.

Consequence: THIS FAILURE MAY INHIBIT THE ABILITY TO DIRECT AIR BLOWING FROM THE HVAC SYSTEM TO THE WINDSHIELD, POTENTIALLY REDUCING THE DRIVER'S VISIBILITY.

Remedy: DEALERS WILL INSPECT THE MODE DOOR OPERATION AND REPLACE THE HVAC CAB MODE HOUSING ASSEMBLY IF NECESSARY. OWNER NOTIFICATION BEGAN NOVEMBER 7, 2001. OWNERS WHO TAKE THEIR VEHICLES TO AN AUTHORIZED DEALER ON AN AGREED UPON SERVICE DATE AND DO NOT RECEIVE THE FREE REMEDY WITHIN A REASONABLE TIME SHOULD CONTACT KENWORTH AT 1-425-828-5000.

01V335000 · Power Train:Driveline:Universal Joint

Reported: 2001-10-25 (October 25, 2001)

VEHICLE DESCRIPTION: HEAVY DUTY TRUCKS. THE DRIVELINE U-JOINT FASTENERS AT THE OUTPUT YOKE OF THE TRANSMISSION WERE IMPROPERLY TIGHTENED.

Consequence: UNDER-TIGHTENED FASTENERS CAN LEAD TO THE DRIVELINE BECOMING DETACHED FROM THE VEHICLE RESULTING IN PROPERTY DAMAGE OR PERSONAL INJURY.

Remedy: DEALERS WILL TIGHTEN THE DRIVELINE FASTENERS TO THE PROPER SPECIFICATION. OWNER NOTIFICATION BEGAN NOVEMBER 26, 2001. OWNERS WHO TAKE THEIR VEHICLES TO AN AUTHORIZED DEALER ON AN AGREED UPON SERVICE DATE AND DO NOT RECEIVE THE FREE REMEDY WITHIN A REASONABLE TIME SHOULD CONTACT KENWORTH AT 1-425-828-5000.

Reported: 2003-04-01 (April 1, 2003)

ON CERTAIN TRUCKS EQUIPPED WITH TRUCK HUB UNITS MANUFACTURED BY SKF EXCLUSIVELY FOR USE ON ARVINMERITOR'S FF98X SERIES NON-DRIVE STEER AXLES, THE HUB UNIT BEARINGS CAN WEAR PREMATURELY.

Consequence: SEIZURE OF THE BEARINGS CAN CAUSE WHEEL SEPARATION OR WHEEL END FIRE.

Remedy: DEALERS WILL REPLACE ALL FRONT AXLE HUBS. NOTIFICATION BEGAN JULY 16, 2003. THE MANUFACTURER HAS ADVISED US THAT PARTS WILL NOT BE AVAILABLE UNTIL SEPTEMBER 2003. OWNERS WHO TAKE THEIR VEHICLES TO AN AUTHORIZED DEALER ON AN AGREED UPON SERVICE DATE AND DO NOT RECEIVE THE FREE REMEDY WITH A REASONABLE TIME SHOULD CONTACT PETERBILT AT 940-591-4201 OR KENWORTH 425-828-5440.

03V168000 · Trailer Hitches:Fifth Wheel Assembly

Reported: 2003-05-05 (May 5, 2003)

ON CERTAIN TRUCKS EQUIPPED WITH JOST INTERNATIONAL JSK 37U SERIES FIFTH WHEELS, A FATIGUE CRACK CAN DEVELOP IN THE BRACKET PIN WELD UNDER CERTAIN LOADING CONDITIONS.

Consequence: IF LEFT UNDETECTED OR CORRECTED IT COULD LEAD TO THE SEPARATION OF THE FIFTH WHEEL TOP PLATE FROM ITS MOUNTING STRUCTURE, POSSIBLY RESULTING IN A CRASH.

Remedy: DEALERS WILL REPLACE THE BRACKET PINS AND REPAIR INSTRUCTIONS FREE OF CHARGE. PETERBILT'S OWNER NOTIFICATION BEGAN JUNE 20, 2003. KENWORTH'S OWNER NOTIFICATION BEGAN JUNE 12, 2003. OWNERS WHO TAKE THEIR VEHICLES TO AN AUTHORIZED DEALER ON AN AGREED UPON SERVICE DATE AND DO NOT RECEIVE THE FREE REMEDY WITHIN A REASONABLE TIME SHOULD CONTACT PETERBILT AT 940-591-4201 OR KENWORTH 425-828-5440.

ON CERTAIN HEAVY DUTY TRUCKS EQUIPPED WITH VERTICAL TAIL PIPES, THE FRAME ACCESS GRABHANDLE IS MOUNTED TOO HIGH.

Consequence: THIS CONDITION COULD CAUSE A PERSON TO SLIP AND/OR FALL AND COULD LEAD TO PERSONAL INJURY.

Remedy: DEALERS WILL INSPECT THE GRABHANDLE LOCATION AND IF DETERMINED TO BE MOUNTED TOO HIGH, THE HANDLE WILL BE RELOCATED TO THE PROPER HEIGHT. OWNER NOTIFICATION BEGAN JULY 29, 2003. OWNERS SHOULD CONTACT KENWORTH AT 1-425-828-5440.

HE THEN RECEIVED RECALL NOTICE # 09V239000 (SERVICE BRAKES, AIR). HOWEVER HE HAS ALREADY PAID AN EXPENSE OF $2500.00 TO REPLACE VARIOUS BRAKING COMPONENTS, AND YET THE FAILURES HAVE NOT BEEN PROPERLY REMEDIED. THE FAILURE MILEAGE WAS 360,000. THE CURRENT MILEAGE WAS 690,000.

TL* THE CONTACT OWNS A 2007 KENTWORTH T600 WITH GOODYEAR SUPER SINGLE TIRES, LINE SIZE 445/50R/22.5 AND THE DOT NUMBER GSWR39965. THE CONTACT STATED THAT WHILE DRIVING APPROXIMATELY 65 MPH THE REAR DRIVER SIDE TIRE BLEW OUT. THE CONTACT REPLACED THE TIRE WITH THE SPARE AND SHORTLY AFTER THE FAILURE RECURRED.
