Recall history

3 distinct campaigns affect this selection. Announcement dates below are different from vehicle model years. A campaign may cover multiple years.

03V120000 · Visibility:Glass, Side/Rear

Reported: 2003-03-27 (March 27, 2003)

CERTAIN TRANSIT BUSES FAIL TO COMPLY WITH THE REQUIREMENTS OF FEDERAL MOTOR VEHICLE SAFETY STANDARD NO. 217, "BUS EMERGENCY EXITS AND WINDOW RETENTION AND RELEASE." THE HANDLE PULL FORCE EXCEEDS THE 89N FORCE TO RELEASE THE EMERGENCY WINDOW. ADDITIONAL FORCE IS REQUIRED TO RELEASE THE WINDOW.

Consequence: IN THE EVENT OF A CRASH, AN OCCUPANT MAY HAVE DIFFICULTY OPENING THE EMERGENCY EXIT WINDOW, INCREASING THE RISK OF PERSONAL INJURY OR DEATH.

Remedy: DEALERS WILL REPAIR THESE WINDOWS. OWNER NOTIFICATION BEGAN JUNE 1, 2003. OWNERS WHO TAKE THEIR VEHICLES TO AN AUTHORIZED DEALER ON AN AGREED UPON SERVICE DATE AND DO NOT RECEIVE THE FREE REMEDY WITHIN A REASONABLE TIME SHOULD CONTACT ORION AT 1-800-492-2116.

04V509000 · Equipment Adaptive/Mobility

Reported: 2004-10-19 (October 19, 2004)

CERTAIN ORION BUSES EQUIPPED WITH SURE-LOK WHEELCHAIR TIE-DOWN SECUREMENT ASSEMBLIES. THE SPROCKET TEETH OF THE RETRACTOR ASSEMBLY MAY BE OUT OF ALIGNMENT CAUSING THE LOAD PAWL NOT TO FULLY SEAT IN THE SPROCKET TEETH.

Consequence: IN THE EVENT OF A VEHICLE CRASH, THE WHEELCHAIR MAY NOT BE ADEQUATELY SECURED POSSIBLY RESULTING IN INJURIES.

Remedy: SURE-LOK WILL CONDUCT THE OWNER NOTIFICATION AND REMEDY FOR THIS CAMPAIGN. DEALERS WILL INSPECT AND REPLACE THE WHEELCHAIR SECUREMENT RETRACTORS AS NECESSARY. OWNERS SHOULD CONTACT SURE-LOK AT 1-866-787-3565 OR ORION AT 905-403-1111.

05V079000 · Fuel System, Diesel

Reported: 2005-03-02 (March 2, 2005)

ON CERTAIN TRANSIT BUSES, THE FUEL LIFT PUMP COULD FAIL IN A MANNER WHICH RESULTS IN ENGINE FUEL STARVATION, WHICH COULD RESULT IN A STALL CONDITION. POTENTIAL LIFT PUMP FAILURE MODES INCLUDE: BRUSH WEAR OUT, BUSHING FAILURE, ROTOR TAB INSERT FAILURE, OR ANY OTHER FAILURE MODE THAT WOULD CAUSE THE FUEL PUMPING ELEMENT TO STOP SPINNING.

Consequence: IF THE FUEL LIFT PUMP PUMPING ELEMENT STOPS SPINNING AND THE FUEL PUMPING VANES STOP IN A LOCATION WHICH BLOCKS FUEL FLOW THROUGH THE LIFT PUMP, THE ENGINE WILL BE STARVED OF FUEL. FUEL STARVATION COULD CAUSE LOW ENGINE POWER, ENGINE MISFIRE, OR ENGINE STALL. PRIOR TO THE ENGINE STALLING, THE OPERATOR COULD EXPERIENCE LOW ENGINE POWER, ENGINE MISFIRE, AND/OR DASH-MOUNTED FAULT LAMP ILLUMINATION.

Remedy: CUMMINS IS CONDUCTING THE OWNER NOTIFICATION AND REMEDY FOR THIS CAMPAIGN. OWNERS MAY CONTACT CUMMINS AT 1-800-343-7357 OR ORION BUS AT 905-403-7807.

Emerging complaint spikes

Complete months in the latest six months with at least 10 complaints, at least twice the preceding 12-month average, and at least three square-root deviations above that average.

This is a descriptive screening rule, not a statistical defect finding. Publicity and reporting changes can increase submissions.

No complete months meet the spike thresholds for this selection.

Equal-age model-year comparison

Model-wide cumulative reports during the first three or five calendar years, starting with the model year (age 0). Only fully elapsed windows are compared.

Incident date is preferred when valid and no later than filing; otherwise filing date is used. Model year approximates age. Equal observation time does not adjust for sales, distance driven or reporting delays. One complaint linked to multiple model years can appear in each cohort.
