I submitted this form earlier, but I’m unsure whether it went through. If this is a duplicate, please disregard. An incident of sudden, unintended acceleration occurred while pulling into a gravel parking area on a hillside at dusk. While turning, the vehicle suddenly lunged forward and struck a tree stump. It would have been avoided without the sudden acceleration. The driver slammed on the brakes, and the vehicle came to rest right at the edge of a downward slope. Five seconds earlier, and a family member, who had come out of the house to sh…
I drove my 2023 TOYOTA BZ4X home from work after a heavy rainfall in Galveston, Texas in January 2024, had about 2-3 inches of rain on the road, driving about 15-20 miles per hour on my way home. Both front wheel wells/mud guard liners came off. I drove over one of the liner, stopped and picked it up. Also a liner in the back of the car's undercarriage was hanging out. Took the car to the dealership the next morning and was told since it's due to external force/rain. The warranty on a 6 months old car is not covered. Called Toyota cooperate of…

22V444000 · Wheels:Hub
Reported: 2022-06-23 (June 23, 2022)
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023 Subaru Solterra and Toyota BZ4X vehicles. The hub bolts on the wheels may loosen, causing a wheel to detach from the vehicle after low-mileage use.
Consequence: A wheel detaching from a moving vehicle can result in a loss of vehicle control, increasing the risk of a crash.
Remedy: Owners are advised not to drive their vehicles until they are repaired. Dealers will replace the hub bolts and wheels, free of charge. Letters notifying owners of the safety risk were mailed June 27, 2022. A second letter will be sent once the remedy is available. Owners may contact Toyota's customer service at 1-800-331-4331.
Gulf States Toyota, Inc. (GST) is recalling certain Toyota 2023 GR Supra, 2024 4 Runner, Corolla, Grand Highlander, Grand Highlander Hybrid, Land Cruiser Hybrid, Tacoma, Tacoma Hybrid, 2023-2024 BZ4X, Corolla Cross Hybrid, GR Corolla, GR86, Highlander, Highlander Hybrid, Prius, Prius Prime, Sequoia Hybrid, Tundra, Tundra Hybrid, Venza Hybrid, 2023-2025 Crown, and 2025 Camry Hybrid vehicles equipped with GST accessories. The load carrying capacity modification label may display inaccurate added weight values. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 110, "Tire Selection and Rims."
Consequence: A vehicle with an incorrect maximum capacity weight value may be overloaded, which can increase the risk of a crash.
Remedy: GST will mail new labels to owners, free of charge. Owner notification letters were mailed September 16, 2024. Owners may contact GST customer service at 1-800-444-1074. GST's number for this recall is 24R2.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023-2025 bZ4X, Lexus RZ, and Subaru Solterra vehicles. During certain HVAC system faults, such as electrical compressor failure, the defroster and defogger system may become inoperative.
Consequence: An inoperative defroster and defogger system can decrease the driver's visibility under certain driving conditions, increasing the risk of a crash.
Remedy: Dealers will update the HVAC control ECU software, free of charge. Dealers will also inspect the electrical compressor, and replace it, if necessary, free of charge. Owner notification letters were mailed October 17, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB07 and 25TA07. Lexus' numbers for this recall are 25LB04 and 25LA04. Subaru's number for this recall is WRD-25.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Consequence: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

On two occasions in the last month, when attempting to start the car, it doesn’t start, the interior dash lights start flickering, and a notification displays stating, “Parking brake cannot be applied”. I then have to “jump” the 12-volt battery. The main issue is the notification stating the parking brake cannot be applied because before shutting off the car the night before, I make sure the parking brake IS applied. However, it seems like the 12-volt battery is dead when I attempt to start it the following day; it automatically turns off the p…

Steering system, potentially related to electric power steering (EPS) or driver-assist systems (lane keeping or steering assist). While completing a routine right-hand turn, my husband began to straighten the steering wheel when the steering system suddenly and sharply turned further right without any driver input. This caused the vehicle to strike a curb, damaging both passenger-side wheels and the front tire. There were no warning lights or indicators, and road and weather conditions were normal. The uncommanded steering input appeared to c…

On [XXX], while driving through a common roadway puddle at a safe speed, the front bumper on my BZ4X detached from the undercarriage shield and tore away from the clips. This type of damage is clearly disproportionate to the conditions and reflects a deficiency in structural design. After researching this issue, I found that many other BZ4X owners have experienced the same bumper detachment and undercarriage shield failure under minimal driving conditions. These consistent reports indicate a widespread manufacturing defect, not isolated driver …

I am writing to register our complaint and urgently request your support in resolving a serious health issue related to what we believe is a factory defect with the Toyota BZ4. Timeline of events: In April 2023, I purchased a brand new BZ4 from the Downtown Oakland Toyota dealership. In July 2024, I noticed a foul smell in the car coming from the air conditioning vents. The car was difficult to drive in due to the severity of the smell. On September 11, 2024, we took the BZ4 to the dealer for maintenance and asked for them to look into th…
AC Compressor. The dealer is saying I need to repair the AC compressor. When this happened my heater and mileage weren't showing and a lot of message errors were showing.
I drove the car at 40mph through about 2 inches of standing water. This caused the undercarriage splash plate to become loose. It has not fallen off, but it will need to be repaired. The streets sometimes flood and none of the cars I’ve owned ever had this happen.

While driving on the highway under normal conditions with no road hazards present, the engine under cover (part #51441-42210) detached from my vehicle and the bumper clips broke, causing both the under cover and bumper to become damaged and requiring replacement.
