While braking there is a brief release, which I believe happens when car switches from regenerative to friction braking. It feels like I have taken my foot off the brake for a moment, but I have not. Result is car briefly stops braking, which can cause loss of control. Has happened consistently for months, less pronounced with hard vs soft braking

DURING ITS FIRST FUEL TANK FILL-UP (AT 909 MILES, ON 3/14/2021), NEW 2021 RAV4 PRIME SE FUEL TANK ACCEPTED LESS THAN THE EXPECTED AMOUNT OF GASOLINE. STARTING AT NEAR-EMPTY, ONLY NINE (9) GALLONS WAS ACCEPTED. AT THE SECOND FILL-UP (ON 3/22/2021), ONLY FOUR GALLONS COULD BE ADDED, AND THE FUEL NOZZLE WAS EJECTED FROM THE TANK, SPILLING GAS. Owner report 11457199 · 2022-03-17 (March 17, 2022)

The RAV4 Prime's fuel system holds 14.5 gallons, however only between 10 - 11 gallons of that is usable per the digital "miles to zero" gauge on the instrument panel. When the gauge reaches "zero" and notifies the driver to fill up, the tank only fills 10 -11 gallons and keeps 3 - 4.5 gallons in reserve. This is a big issue because the "miles to zero" gauge reads the full tank when you fill it up (approximately 550 miles), but begins to swing drastically on miles left as you drive the car to accommodate for the 10 - 11 gallons that it actuall…

Something needs to be done because Toyota is stalling on sending out a critical safety component that has been recalled in thousands of vehicles. It is the HV floor wire connector, when the corrosion is beyond repair, the parts have been back ordered for months. Toyota and dealerships are not approving loaner vehicles. This is a serious safety issue because: If the wire corrodes or shorts out, the vehicle can experience unexpected power loss, stalling, or complete hybrid system failure. this could happen in the middle of driving in fast, traffi…

The 2021 RAV4 Prime has a known defect with the heat pump causing an alarming sound to be heard within about a 1 minute of startup. The grinding noise causes distraction as it is extremely loud and occurs generally while backing out/maneuvering in tight areas. Toyota released a Service Bulletin in their European Market in October (numbered BE-00612T-TME - "Train Horn noise" from A/C compressor when the vehicle is started), but has not made available the same fixes in the US even months later.

The primary instrument display started to go out when it was cold, below 50'F. This could be mitigated by running the engine or running the defrost for a period of time to warm up the area around the display. The Toyota pre-collision braking system is too slow to release control of the throttle back to the driver. Multiple times it has made the car behave in an unexpected fashion in traffic. The most common scenario is when the driver is stopped and turning left onto a busy street. When a car passes in front of you and you try to accelerate after it passes, the pre-collision braking still thinks there's a car in front of you. Instead of acceleration your car just gently rolls into the street. The driver can't do anything in the moment to override…

This sudden explosive noise put the driver, and all other vehicles on the road, in danger due to the panicked reflex of the driver. I was driving along an expressway on a rainy night. The plug-in hybrid car was operating on battery power. Suddenly the motor went dead and it slowed to a stop. This was very dangerous because the expressway traffic had to swerve around me. I may have seen a hybrid system malfunction warning on the display, but I was so distraught that I didn't think to take a photo. After about 5 minutes, I was able to start the car and drive away. The Toyota dealer examined the car later and could find nothing wrong.

On July 24th, I experienced sudden unexpected acceleration with my 2021 Toyota Rav4 Prime XSE (41,600 miles) while driving in the electric mode. I was lining myself up to reverse into my garage and as I paused to switch gears to reverse, the car suddenly accelerated full-speed towards my neighbor's house. I slammed the brakes but that had little effect. The car slammed into the house, through the garage door and my car was only stopped when hitting an SUV parked in the garage, pushing it forward about 2 feet. No air bags were deployed, the …

It does not happen consistently, but it does happen fairly regularly (every 2-4 weeks). Note, the grinding sound can range from subtle to quite loud, depending on bump/timing of braking. This has been happening since I purchased the car and continues to happen, which is why I put today's date as the incident date, and I have been bringing it up with the Toyota maintenance department at most maintenance appointments. The vehicle generated an error message "Plug In Charging System Malfunction" warning on the dash. The remedy is a Toyota T-SB-0021-21 which requires a software upgrade to fix the air fan sensor reading to the charging unit. This is only considered part of the 60 month/60,000 mile power train warranty despite it being part of the battery charging system which has a 150,000 mile warranty. It is also presumably caused by a software error Toyota has made. They should fix this at no cost to the customer, instead I was charged $215 plus tax.

My 2021 Toyota RAV4 Prime had traction control, braking issues and failure to warn in the event of a possible forward collision. On a few occasions the brakes seem to just slide and the car would keep moving. Last week I was in an accident and the emergency braking system, the forward collision warning system and all the air bags failed. The California Highway Patrol was surprised that even though the estimation collision speed was 60 mph the air bags did not deploy. I would have not had a chance to test out the air bags, emergency braking syst…

MY [XXX] ’S) SUDDEN ACCELERATION INCIDENT [XXX] - TOYOTA RAV4 2021: I was gliding into a parking space with my foot on the brake and as I went to straighten out the wheel to come to a full stop the car lurched forward full speed and smashed into the car parked in the row that was facing me. Good thing there was no one in that car, it had been backed in so I hit the rear end of it. The damage is so extensive it is beyond comprehension. My airbags did not deploy, which they should have. I saw that the ‘malfunction brake’ orange light was ill…

I applied the applied the breaks hard and slid into the passenger side of the turning vehicle. The ABS system did not function. The air bag did not deploy. At the moment of the collision my speed was reduced and I do not know the threshold for airbags to deploy, but this was a direct front end collision. I was able to brace myself and was not injured. My RAV4 PRIME was in stop and go traffic. Brakes did not switch from regen braking to physical brakes. Slammed on the brakes several times, they did not slow the car enough. Eventually the brakes locked up as the RAV4 rear ended the car in front. Pushing that car into the one in front of it, and that one in front of that, and that one in front of that. Causing damage to three other cars. Two other people are claiming medical, the driver went to the ER. Air bags did not deploy. Taking to the repair shop. They were shocked that the air bags did…

Toyota issued a recall in July 2023 stating the the DC-DC conversion system may have been damaged in manufacture, potentially causing a short, and possibly leading to a fire if charged at less than 41 degrees. Toyota indicated that a fix would be available in the 4th quarter of 2023. We are nearing the end of the 4th quarter and fix is still not available. Living in upstate New York, nighttime temperatures frequently are less than 41 degrees. As a result I am reluctant to charge overnight. This defeats the purpose of a plug-in hybrid, resu…

While exiting an interstate and applying the brakes, the vehicle struck a small bump (pot-hole) in the roadway with the front passenger wheel. The car lurched forward and stopped slowing down. The brakes had disengaged on all wheels. After approximately 2 seconds, the car started braking again and I was able to regain control and stop.
