The 12.3-inch display of the instrument panel went blank when I was driving and away from home. Operating the vehicle with this display blank resulted in usafe diriving conditions including inability to see speed, gas tank status, charge status and all malfunction indicators. Since the malfunction occured while driving, I was at an increased risk of a crash or an increased risk of injury in the event of a crash. I had to drive home and then later to the dealership with this malfunction occuring.

The 2021 RAV4 Prime has a known defect with the heat pump causing an alarming sound to be heard within about a 1 minute of startup. The grinding noise causes distraction as it is extremely loud and occurs generally while backing out/maneuvering in tight areas. Toyota released a Service Bulletin in their European Market in October (numbered BE-00612T-TME - "Train Horn noise" from A/C compressor when the vehicle is started), but has not made available the same fixes in the US even months later.

The primary instrument display started to go out when it was cold, below 50'F. This could be mitigated by running the engine or running the defrost for a period of time to warm up the area around the display. When it was too cold this winter, that was no longer effective and the display would not come on at all. The Toyota pre-collision braking system is too slow to release control of the throttle back to the driver. Multiple times it has made the car behave in an unexpected fashion in traffic. The most common scenario is when the driver is stopped and turning left onto a busy street. When a car passes in front of you and you try to accelerate after it passes, the pre-collision braking still thinks there's a car in front of you. Instead of acceleration your car just gently rolls into the street. The driver can't do anything in the moment to override…

Panoramic moon roof exploded while driving with no impact to the glass. The explosion sounded like a gun shot inside the car. This sudden explosive noise put the driver, and all other vehicles on the road, in danger due to the panicked reflex of the driver. Upon inspection, it's evident that the glass shattered along all 4 edges rather than from a single point of impact. This would indicate an issue with the glass, the installation, or both. Vehicle was taken to dealer and customer was told it would not be covered under warranty.

I was pulling slowly into my driveway and suddenly the car began accelerating on it's own. I braked and it would not stop. I ended up running into my electronic gate and hitting the side of my neighbor's house. Safety was put at risk because I could have hit a person or moving car had this not happened in my driveway. I could have caused more substantial damage to my neighbor's house. I will report this to the dealership and have it inspected. There were no warning lamps or messages or warning symptoms.

I slowly turned right into a parking space, braked to stop but the car didn't stop and instead lurched, jumping forward over the parking curb and hitting a small wall. My foot was on the brake pedal the entire time. It happened at 9:00am on a clear day. I was alone in the car and uninjured. Damage to the car was extensive and estimated at nearly $40,000. As a result, car was totaled by the insurance company.

The primary instrument display started to go out when it was cold, below 50'F. This could be mitigated by running the engine or running the defrost for a period of time to warm up the area around the display. When it was too cold this winter, that was no longer effective and the display would not come on at all. The car would drive and all of the instruments displayed on the screen would work (cruise control, etc.), but there was no way to know how fast I was going and if there were any warning lights illuminated. On Friday Jan 30 around 3pm, this Rav 4 entered the short drive way in front of the garage. The car stopped, the automatic garage door was engaged. Suddenly, the car advanced, passing through the opening garage door and into and through the back wall of the garage. This is a sustaining way for the second storey. The front of the car displaced the counters inside, demolished the kitchen, almost toppling the fridge. Given the damage to the garage done and the damage inflicted, my interpretation is that an unintended acceleration event occurred.…

The instrument cluster (combination meter) failed completely and without warning, going entirely blank. This eliminated all speedometer, warning light, and indicator functionality. The failure began occurring intermittently before becoming persistent. No warning lamps or messages preceded the failure , the cluster simply went dark. The loss of speedometer functionality made the vehicle unsafe to operate, as the driver had no means of monitoring vehicle speed. The loss of all warning indicators eliminated visibility into any safety-critical vehi…

Driver was parking the car in a parking lot at a very slow speed. The car suddenly shot forward at a very high rate of speed, requiring evasive steering from trees and a fence, until the car crashed head-on into another vehicle. The driver is very certain the brakes only were applied during the entire time the car shot forward. Airbags were not deployed. Bystanders were within six feet of the car at one point during the acceleration, prior to the crash. There were no injuries. Earlier, on the day of the crash, the car had a sudden, stro…

Running the car long enoug…

The digital dashboard (instrument cluster)on my 2023 Toyota RAV4 Prime, suddenly went completely black. This included the speedometer, warning lights, fuel gauge, and all other driver information. I was unable to monitor my speed, vehicle status, or receive safety alerts , a serious risk while operating the vehicle. I have not authorized repairs yet. I brought the issue to Toyota’s attention, but since the vehicle is out of warranty and over the mileage limit, the dealership is requiring a $200 diagnostic fee just to evaluate the problem. Ther…
