Brake system Fault - no power assist. While the Deputy was driving his vehicle every light on the dash lit up and it said all systems were not working. The vehicle was taken to a local dealer and they are looking to replace the ABS HCU on this vehicle. Recall history

2 distinct campaigns affect this selection. Announcement dates below are different from vehicle model years. A campaign may cover multiple years.

25V691000 · Forward Collision Avoidance: Warnings:External/Pedestrian Alert | Back Over Prevention:Warnings:External/Pedestrian Alert

Reported: 2025-10-10 (October 10, 2025)

Ford Motor Company (Ford) is recalling certain 2024-2026 Lincoln Nautilus Hybrid and 2025 Explorer hybrid police vehicles. A software error may prevent the vehicle from emitting the pedestrian warning sound at certain speeds.

Consequence: A vehicle that does not produce a warning sound may fail to alert pedestrians of an approaching vehicle, increasing the risk of injury.

Remedy: The audio control module and digital signal processing module software will be updated by a dealer, or through an over-the-air (OTA) update, free of charge. Owner notification letters were mailed November 5, 2025. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 25SA2. Vehicle Identification Numbers (VINs) involved in this recall became searchable on NHTSA.gov on October 10, 2025.

Reported: 2026-06-30 (June 30, 2026)

Ford Motor Company (Ford) is recalling certain 2024-2027 Lincoln Nautilus Hybrid and 2025-2027 Explorer Hybrid vehicles. A software error may prevent the vehicle from making the pedestrian warning sound at certain speeds.

Consequence: A vehicle that does not produce a warning sound may fail to alert pedestrians of an approaching vehicle, increasing the risk of injury.

Remedy: Note: This is an expansion of Recall 25V691. Any vehicles previously repaired under 25V691 must have the new remedy completed. Ford will release an over-the-air (OTA) update or dealers will update the APIM software, free of charge. Additionally, dealers will replace the digital signal processing module in Nautilus Hybrid vehicles equipped with 28 speakers, free of charge. This will be a phased campaign, with the remedy becoming available in different phases based on model and model years. Owner notification letters were mailed August 7, 2026. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 26S51. A Police Officer's Ford Explorer Interceptor Hybrid SUV while driving had a vehicle malfunction where the rear wheels began to wobble/vibrate and while driving in a straight line with front wheels facing straight caused the vehicle to fishtail out of the control (loss of control to the rear wheels) and slam into a tree at approximately 50mph injuring the officer. Officer's injuries were broken left leg and two knee fractures. Upon further research, this incident may have been caused by the "rear bolt" fracture issue Ford is having with these …

I was driving the vehicle through a neighborhood and there was a clunk noise and it sounded like my brakes were metal on metal. I stopped to ensure my brakes were okay. Then I continued to drive home when there was a louder clunk and the reared on my vehicle locked up and the car would no longer move. The reared differential unit had failed and exploded parts and put holes in the unit causing fluid to exit onto the street and lock up the drive tires. The vehicle was stranded in the middle of the street and could not move. There was no warning o…

I was making a slow speed u-turn to stop a speeding motorist. As I came out of the u-turn, the wheel did not come back to center and I continued the turn going up a curb and striking a chain link fence. When I realized what was occurring, I manually began to turn the steering wheel to the right to get back on the roadway. There is patrol car dash cam footage of this collision. Police report# 23-688.

Twice the car has blown the main 80 amp fuse for the electric power steering after one of the front wheels hit something that caused the wheel to jerk slightly. I did not think either of these bumps should have caused the power steering to go out. one of them occurred at very low speed maybe two or three miles per hour. Neither of them were sufficient to cause any damage to any of the mechanical parts. But in both cases the power steering completely went out which makes the car very difficult and dangerous to drive. I suspect this is a common p…

On July 8, 2026, I was traveling westbound on Highway 7 when another vehicle pulled right out in front of me from southbound Orchard Road, attempting to cross the highway. We collided and I struck the other vehicle at a 45° angle. My seatbelt was buckled. The impact caused me to be thrown forward into the windshield which I broke with my skull. I suffered a broken neck. The seatbelt did not operate properly to keep me secure in the seat. There's no way I should've struck the windshield if the seatbelt would've worked. Not only did the shoulder …

Emerging complaint spikes

Complete months in the latest six months with at least 10 complaints, at least twice the preceding 12-month average, and at least three square-root deviations above that average.

This is a descriptive screening rule, not a statistical defect finding. Publicity and reporting changes can increase submissions.

No complete months meet the spike thresholds for this selection.
