Recall history

1 distinct campaigns affect this selection. Announcement dates below are different from vehicle model years. A campaign may cover multiple years.

15V771000 · Equipment:Recreational Vehicle/Trailer:Lpg Systems:Water Heater

Reported: 2015-11-19 (November 19, 2015)

Forest River, Inc. (Forest River) is recalling certain model year 2012-2016 Berkshire, Charleston, Legacy, Dynamax, XLR, Continental Cargo, Work & Play, Shockwave, and Coachmen Brookstone recreational vehicles and toy haulers, equipped with Atwood on demand water heaters, models OD45, OD50, and OD50CW, manufactured January 2, 2012, to August 13, 2015. The affected water heaters have a water and gas valve that may cause the water heater to overheat the water in the hot water supply tap.

Consequence: The overheating of the water may result in the release of steam, possibly causing the operator is to scald themselves and increasing the risk of injury.

Remedy: Forest River will notify owners of a field remedy as provided by Atwood to the Dealerships and O.E.M. A. Inspection and installation instructions for the remedy, to be used and applied only by qualified service center personnel. B. Installation of a water flow adjustment STOP KEY that prevents the adjustment of the water flow down below .5gal/min. C. The addition of a star or lock washer to secure the adjustment screw that prevents unintended adjustment or and adjustment drift from vibration as you travel down the road. D. Securement of the remedy with the application of a warning and tamper proof label applied over the stop key and adjustment screw. The recall began on December 17, 2015. Owners may contact Forest River customer service at 1-800-348-7440, or Atwood customer service at 1-574-264-2131. Forest River's number for this recall is 51-11092015-0107.

The tire which was damaged was a rear passenger tire on a travel trailer, it was monitored by TPMS and was performing as expected showing no signs of air loss or temperature rise - it was a sudden explosion and the tread completely came off the tire causing damage to the trailer and of course the need for roadside assistance. Upon inspection of the remaining tires in the group there found to be sidewall cracks. Tire size was P225/75R15.

TIRE BLEW OUT. TRAILER WAS LESS THAN A YEAR OLD. CHECK AIR PRESSURE EACH TRIP AND USE IT ONCE A MONTH, SO NOT SITTING TOO LONG. TRAILER GVWR IS WHAT THE 4 TIRES CARRIED AND THE ESTIMATED TONGUE WEIGHT ADDED TOGETHER. SHOULD HAVE HIGHER RATED TIRES FOR SAFETY MARGIN. BEEN READING ONLINE ABOUT MANY OF THESE TIRES BLOWING OUT. THE BLOW OUT OCCURRED ON I 10 RETURNING FROM A CAMP OUT ON A STRAIGHT SECTION NEAR US 301. I WAS TRAVELING ABOUT 65 IN THE RIGHT LANE. SUDDEN BOOM AND I PULLED OVER TO INVESTIGATE.

Emerging complaint spikes

Complete months in the latest six months with at least 10 complaints, at least twice the preceding 12-month average, and at least three square-root deviations above that average.

This is a descriptive screening rule, not a statistical defect finding. Publicity and reporting changes can increase submissions.

No complete months meet the spike thresholds for this selection.

Equal-age model-year comparison

Model-wide cumulative reports during the first three or five calendar years, starting with the model year (age 0). Only fully elapsed windows are compared.
