Recall history
3 distinct campaigns affect this selection. Announcement dates below are different from vehicle model years. A campaign may cover multiple years.
12V021000 · Latches/Locks/Linkages:Doors:Latch
Reported: 2012-01-19 (January 19, 2012)
Morgan Olson is recalling certain model year 2009-2011 walk-in van type vehicles with a gross vehicle weight rating in excess of 4,536kg (10,000lbs), manufactured from September 1, 2009, through January 19, 2012, with sliding front doors. These vehicles do not have either a primary door latch system, or a door closure warning system. Therefore, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard No. 206, "Door Locks and Door Retention Components."
Consequence: Without either a primary door latch system or a door latch system with a fully latched position and a door closure warning system as required by FMVSS 206, the door could partially open without warning and the driver may not be aware that the door is not fully latched, putting the driver at an increased risk of injuries in the event of a crash.
Remedy: Morgan Olson will notify owners, and dealers will repair the vehicles free of charge. The safety recall began on April 25, 2012. Owners may contact Morgan Olson at 1-269-659-0200.
NHTSA campaign record ↗12V045000 · Latches/Locks/Linkages:Doors:Latch
Reported: 2012-02-07 (February 7, 2012)
CERTAIN MODEL YEAR 2009-2012 MORGAN OLSON WALK-IN VAN TYPE VEHICLES, MANUFACTURED FROM SEPTEMBER 1, 2009, THROUGH FEBRUARY 2, 2012, WITH SLIDING FRONT SIDE DOORS, HAVE DOOR LATCH SYSTEMS THAT FAIL TO MEET THE STRENGTH REQUIREMENTS OF S4.2.1.1(a) OF FEDERAL MOTOR VEHICLE SAFETY STANDARD NO. 206, "DOOR LOCKS AND DOOR RETENTION COMPONENTS," AND THEREFORE DO NOT COMPLY.
Consequence: A DOOR LATCH SYSTEM THAT DOES NOT COMPLY WITH STRENGTH REQUIREMENTS MAY NOT REMAIN FULLY LATCHED KEEPING THE DOOR CLOSED, ESPECIALLY IN THE EVENT OF A CRASH, AND MAY INCREASE THE POSSIBILITY OF OCCUPANT PARTIAL OR FULL EJECTION.
Remedy: MORGAN OLSON WILL NOTIFY OWNERS, AND THE VEHICLES WILL BE REPAIRED FREE OF CHARGE. THE SAFETY RECALL BEGAN ON APRIL 25, 2012. OWNERS MAY CONTACT MORGAN OLSON AT 1-269-659-0200.

Reported: 2012-09-03 (September 3, 2012)
Morgan Olson is recalling certain model year 2009 through 2011 walk-in trucks due to failure to conform to various Federal Motor Vehicle Safety Standards (FMVSS). These vehicles may fail to comply with one or more of the following: FMVSS No. 101, "Controls and Displays;" FMVSS No. 103, "Windshield Defrosting and Defogging Systems;" FMVSS No. 104, "Windshield Wiping and Washing Systems;" FMVSS No. 108, "Lamps, Reflective Devices, and Associated Equipment;" FMVSS No. 206, "Door Locks and Door Retention Components;" and 49 CFR Part 567 "Certification."
Consequence: Failure of the vehicles to meet one or more required safety standards may increase the risk of a crash or personal injury.
Remedy: The only owner of the affected vehicles has been notified and Morgan Olson is working with the owner to bring the vehicles into compliance. Therefore, a owner notification letter will not be issued for this campaign. Any repairs will be made free of charge.

Emerging complaint spikes
Complete months in the latest six months with at least 10 complaints, at least twice the preceding 12-month average, and at least three square-root deviations above that average.
This is a descriptive screening rule, not a statistical defect finding. Publicity and reporting changes can increase submissions.
No complete months meet the spike thresholds for this selection.
Equal-age model-year comparison
Model-wide cumulative reports during the first three or five calendar years, starting with the model year (age 0). Only fully elapsed windows are compared.
2011: 0 complaints · 2012: 0 complaints

Date coverage across each selected cohort

|---|---|---|---|
| 2011 | 0 | 0 | 0 |
| 2012 | 0 | 0 | 0 |
Incident date is preferred when valid and no later than filing; otherwise filing date is used. Model year approximates age. Equal observation time does not adjust for sales, distance driven or reporting delays. One complaint linked to multiple model years can appear in each cohort.
